Kickstarted Video Game Console Gets Ready for Debut - Hit & Run ...

The next Super Nintendo or the next Atari Jaguar?Source: OuyaA new video game console is about to hit the stores, and it?s not produced by Sony, Microsoft or Nintendo, the current gorillas dominating the market.

Ouya, a much more modest $99 console, running off Android?s operating system, will be hitting the market in June and will be available in stores like Target and Best Buy.

The launch of Ouya will mark the culmination of an interesting experiment. The completion of the console?s development and release was paid for via a Kickstarter campaign that drew in $8.5 million in donations. Kickstarter campaigns have been successful in helping fund products and artistic ventures. But as gamers know, the days of a console being just a product are long over. Game consoles also now provide persistent services. Games can be purchased and downloaded via console these days, as can rentals of movies and television shows. ?

Ouya?s challenge will be not just getting the console into gamers? hands, but in being able to continue to operate an online market given its economically modest origins. I wrote about Ouya back in August and the skepticism over whether such a business model could possibly succeed.

Chris Kohler at Wired got his hands on the console in San Francisco just recently. It obviously is not going to be competing with the Xbox or the Playstation for the high-end games, but that?s absolutely not the point of the system. Rather, it intends to be an avenue for smaller-game developers to more easily find an audience. Because of the huge boom of indie game development, the success of the system will depend on the ability for creators to use their marketplace and the ease of consumers in connecting to the types of games they?d like. Kohler notes the system Ouya will be using for its marketplace incorporates both an algorithm and human curation:

It?s common knowledge in the world of iOS apps that you get noticed in one of two ways: Get featured in the store via Apple?s secretive process of internal curation, or (by hook or by crook) get onto the top-grossing or most-downloaded charts.

?We don?t think downloads or revenue are good indicators of what a good game is,? [Ouya CEO Julie] Uhrman says. To that end, Ouya is crafting its own automatic algorithm that will determine whether or not a game is any good, based on other players? behaviors. How many times have they played it? For how long are they playing it? When a player turns on their Ouya, is it the first game they immediately boot up? All of these factors will influence how prominently games are positioned in the Ouya marketplace when a player clicks on ?Discover.?

There will also be an element of hand-picked curation on Ouya. That process, Uhrman says, will be led by Kellee Santiago, co-founder Journey creator thatgamecompany and now Ouya?s head of developer relations. All new games will go into an area called the ?Sandbox,? and will be pulled up into the ?Recommended? feed after they hit the jackpot on the automated fun algorithm, or are selected by Santiago?s team.

There?s still plenty of skepticism among gamers that Ouya?s business model can actually work (just read the comments under Kohler?s story). Portability and simplicity has contributed to the success of mobile phone gaming apps. Do people really want to play such games on their televisions? We will find out beginning in June. For the curious, PC Magazine has a slide show of some games that will be available at launch via Ouya.

Even if Ouya fails, console developers will likely learn a lot about building economic systems that accommodate the ever-changing nature of the gaming marketplace.

Two fifth-graders face trial in alleged Washington murder plot

SEATTLE (Reuters) - Two Washington state fifth-grade boys, accused in a foiled plot to rape and kill a girl and kill or harm six other classmates, will stand trial as juveniles, a prosecutor said on Friday.

Stevens County Prosecutor Tim Rasmussen said the boys, 11 and 10, pleaded not guilty during an arraignment on Friday over the alleged murder-rape plot that also targeted other children in Colville, Washington, about 215 miles east of Seattle.

One of the boys wanted the girl dead because "she's rude and always made fun of me and my friends," according to court documents.

"There are very few prosecutions of a crime of this magnitude with boys of this age," Rasmussen told Reuters.

Stevens County Superior Court Judge Allen Nielson determined during the hearing that the boys had the capacity to understand right and wrong, Rasmussen said.

Under Washington state law, children ages 8 to 12 are presumed not to have the mental capacity to form an intention to commit a crime. Juvenile court is typically reserved for defendants between ages 12 and 18.

The 10-year-old suspect was charged with conspiracy to commit first-degree murder, juvenile firearm possession and witness tampering, Rasmussen said.

The younger boy had taken a gun that originally belonged to his grandfather from his older brother's room, according to a declaration of probable cause.

The 11-year-old was charged with conspiracy to commit first-degree murder, possession of a dangerous weapon, a knife, at school and tampering with a witness, Rasmussen said.

The boys planned to lure the girl away from school in Colville, a town of 4,600 people in eastern Washington, according to court documents.

A teacher found a list of six more targeted classmates, prosecutors said.

The fifth-graders had boarded a school bus on February 7 with a knife, a semi-automatic pistol and ammunition in a backpack on their way to Fort Colville Elementary School, prosecutors said.

A fourth-grader on the bus saw the knife and reported it to a teacher's aide, prosecutors said. One of the suspects later said he would kill the student who told school officials about the weapons, they said.

If convicted of all charges, the boys could each be sentenced to 103 to 127 weeks unless a judge finds "manifest injustice" and orders a longer sentence, Rasmussen said.

The boys have been expelled from the school district indefinitely. A status hearing is set for April 8.

It's a bird, it's plane...no, it's a drone

WASHINGTON (AP) ? It's a good bet that in the not-so-distant future aerial drones will be part of Americans' everyday lives, performing countless useful functions.

A far cry from the killing machines whose missiles incinerate terrorists, these generally small, unmanned aircraft will help farmers more precisely apply water and pesticides to crops, saving money and reducing environmental impacts. They'll help police departments find missing people, reconstruct traffic accidents and act as lookouts for SWAT teams. They'll alert authorities to people stranded on rooftops by hurricanes and monitor evacuation flows.

Real estate agents will use them to film videos of properties and surrounding neighborhoods. States will use them to inspect bridges, roads and dams. Oil companies will use them to monitor pipelines, while power companies use them to monitor transmission lines.

With military budgets shrinking, drone makers have been counting on the civilian market to spur the industry's growth. But there's an ironic threat to that hope: Success on the battlefield may contain the seeds of trouble for the more benign uses of drones at home.

The civilian unmanned aircraft industry worries that it will be grounded before it can really take off because of fear among the public that the technology will be misused. Also problematic is a delay in the issuance of government safety regulations that are needed before drones can gain broad access to U.S. skies.

Some companies that make drones or supply support equipment and services say the uncertainty has caused them to put U.S. expansion plans on hold, and they are looking overseas for new markets.

"Our lack of success in educating the public about unmanned aircraft is coming back to bite us," said Robert Fitzgerald, CEO of The BOSH Group of Newport News, Va., which provides support services to drone users.

"The U.S. has been at the lead of this technology a long time," he said. "If our government holds back this technology, there's the freedom to move elsewhere ... and all of a sudden these things will be flying everywhere else and competing with us."

Since January, drone-related legislation has been introduced in more than 30 states, largely in response to privacy concerns. Many of the bills are focused on preventing police from using drones for broad public surveillance, as well as targeting individuals for surveillance without sufficient grounds to believe they were involved in crimes.

Law enforcement is expected to be one of the bigger initial markets for civilian drones. Last month, the FBI used drones to maintain continuous surveillance of a bunker in Alabama where a 5-year-old boy was being held hostage.

In Virginia, the state General Assembly passed a bill that would place a two-year moratorium on the use of drones by state and local law enforcement. The measure is supported by groups as varied as the American Civil Liberties Union on the left and the Virginia Tea Party Patriots Federation on the right.

Gov. Bob McDonnell is proposing amendments that would retain the broad ban on spy drones but allow specific exemptions when lives are in danger, such as for search-and rescue operations. The legislature reconvenes on April 3 to consider the amendments.

"Any legislation that restricts the use of this kind of capability to serve the public is putting the public at risk," said Steve Gitlin, vice president of AeroVironment, a leading maker of smaller drones, including some no bigger than a hummingbird

Seattle abandoned its drone program after community protests in February. The city's police department had purchased two drones through a federal grant without consulting the city council.

Drones "clearly have so much potential for saving lives, and it's a darn shame we're having to go through this right now," said Stephen Ingley, executive director of the Airborne Law Enforcement Association. "It's frustrating."

In some states economic concerns have trumped public unease. In Oklahoma, an anti-drone bill was shelved at the request of Republican Gov. Mary Fallin, who was concerned it might hinder growth of the state's drone industry. The North Dakota state Senate killed a drone bill in part because of concern that it might impede the state's chances of being selected by the Federal Aviation Administration as one of six national drone test sites, which could generate local jobs.

A bill that would have limited the ability of state and local governments to use drones died in the Washington legislature. The measure was opposed by The Boeing Co., which employs more than 80,000 workers in the state and which has a subsidiary, Insitu, that's a leading military drone manufacturer.

Although the Supreme Court has not dealt directly with drones, it has OK'd aerial surveillance without warrants in drug cases in which officers in a plane or helicopter spotted marijuana plants growing on a suspect's property. But in a case involving the use of ground-based equipment, the court said police generally need a warrant before using a thermal imaging device to detect hot spots in a home that might indicate that marijuana plants are being grown there.

In Congress, Rep. Ed Markey, D-Mass., co-chairman of the House's privacy caucus, has introduced a bill that prohibits the Federal Aviation Administration from issuing drone licenses unless the applicant provides a statement explaining who will operate the drone, where it will be flown, what kind of data will be collected, how the data will be used, whether the information will be sold to third parties and the period for which the information will be retained.

Sentiment for curbing domestic drone use has brought the left and right together perhaps more than any other recent issue. "The thought of government drones buzzing overhead and constantly monitoring the activities of law-abiding citizens runs contrary to the notion of what it means to live in a free society," Sen. Charles Grassley, R-Iowa, said at a recent hearing of the Senate Judiciary Committee.

Privacy advocates acknowledge the many good uses of drones. In Mesa County, Colo., for example, an annual landfill survey using manned aircraft cost about $10,000. The county recently performed the same survey using a drone for about $200.

But drones' virtues can also make them dangerous, they say. Their low cost and ease of use may encourage police and others to conduct the kind of continuous or intrusive surveillance that might otherwise be impractical. Drones can be equipped with high-powered cameras and listening devices, and infrared cameras that can see people in the dark.

"High-rise buildings, security fences or even the walls of a building are not barriers to increasingly common drone technology," Amie Stepanovich, director of the Electronic Privacy Information Council's surveillance project, told the Senate panel.

Civilian drone use is limited to government agencies and public universities that have received a few hundred permits from the FAA. A law passed by Congress last year requires the FAA to open U.S. skies to widespread drone flights by 2015, but the agency is behind schedule and it's doubtful it will meet that deadline. Lawmakers and industry officials have complained for years about the FAA's slow progress.

The FAA estimates that within five years of gaining broader access about 7,500 civilian drones will be in use.

Sen. Rand Paul, R-Ky., recently drew attention to the domestic use of drones when he staged a Senate filibuster, demanding to know whether the president has authority to use weaponized drones to kill Americans on American soil. The White House said no, if the person isn't engaged in combat. But industry officials worry that the episode could temporarily set back civilian drone use.

"The opposition has become very loud," said Gitlin of AeroVironment, "but we are confident that over time the benefits of these solutions (drones) are going to far outweigh the concerns, and they'll become part of normal life in the future."

Common gene variants explain 42% of antidepressant response

Mar. 28, 2013 ? Antidepressants are commonly prescribed for the treatment of depression, but many individuals do not experience symptom relief from treatment. The National Institute of Mental Health's STAR*D study, the largest and longest study ever conducted to evaluate depression treatment, found that only approximately one-third of patients responded within their initial medication trial and approximately one-third of patients did not have an adequate clinical response after being treated with several different medications. Thus, identifying predictors of antidepressant response could help to guide the treatment of this disorder.

A new study published in Biological Psychiatry now shares progress in identifying genomic predictors of antidepressant response.

Many previous studies have searched for genetic markers that may predict antidepressant response, but have done so despite not knowing the contribution of genetic factors. Dr. Katherine Tansey of Institute of Psychiatry at King's College London and colleagues resolved to answer that question.

"Our study quantified, for the first time, how much is response to antidepressant medication influenced by an individual's genetic make-up," said Tansey.

To perform this work, the researchers estimated the magnitude of the influence of common genetic variants on antidepressant response using a sample of 2,799 antidepressant-treated subjects with major depressive disorder and genome-wide genotyping data.

They found that genetic variants explain 42% of individual differences, and therefore, significantly influence antidepressant response.

"While we know that there are no genetic markers with strong effect, this means that there are many genetic markers involved. While each specific genetic marker may have a small effect, they may add up to make a meaningful prediction," Tansey added.

"We have a very long way to go to identify genetic markers that can usefully guide the treatment of depression. There are two critical challenges to this process," said Dr. John Krystal, Editor of Biological Psychiatry. "First, we need to have genomic markers that strongly predict response or non-response to available treatments. Second, markers for non-response to available treatments also need to predict response to an alternative treatment. Both of these conditions need to be present for markers of non-response to guide personalized treatments of depression."

"Although the Tansey et al. study represents progress, it is clear that we face enormous challenges with regards to both objectives," he added. "For example, it does not yet appear that having a less favorable genomic profile is a sufficiently strong negative predictor of response to justify withholding antidepressant treatment. Similarly, there is lack of clarity as to how to optimally treat patients who might have less favorable genomic profile.."

Additional research is certainly required, but scientists hope that one day, results such as these can lead to personalized treatment for depression.

RIM success in 4Q, but too early to declare win

TORONTO (AP) ? Research In Motion Ltd., once written off as dead amid fierce competition from more modern mobile devices such as the iPhone, surprised Wall Street Thursday by returning to profitability and shipping more BlackBerry 10 phones than expected in the most recent quarter.

It will take several quarters, though, to know whether RIM is on a path toward a successful turnaround. RIM just entered the crucial U.S. market with the new phone last week. And despite selling a million BlackBerry 10 phones in other countries, RIM lost subscribers for the second consecutive quarter.

Thursday's earnings report provided a first glimpse of how the BlackBerry 10 system, widely seen as crucial to the company's future, is selling internationally and in Canada since its debut Jan. 31. The 1 million new touch-screen BlackBerry Z10 phones were above the 915,000 that analysts had been expecting for the quarter that ended March 2. Details on U.S. sales are not part of the fiscal fourth quarter's financial results because the Z10 wasn't available there after the quarter ended.

Investors appeared happy with the financial results. RIM's stock rose 34 cents, or 2.4 percent, to $14.91 in afternoon trading Thursday after the release of results. Many analysts had written RIM off last year, but now believe the Canadian company has a future.

"I thought they were dead. This is a huge turnaround," Jefferies analyst Peter Misek said from New York.

Misek said the Canadian company "demolished" the numbers, especially its gross margins. RIM reported gross margins of 40 percent, up from 34 percent a year earlier. The company credited higher average selling prices and higher margins for devices.

"This is a really, really good result," Misek said. "It's off to a good start."

The new BlackBerry 10 phones are redesigned for the new multimedia, Internet browsing and apps experience that customers are now demanding.

The BlackBerry, pioneered in 1999, had been the dominant smartphone for on-the-go business people and other consumers before the iPhone debuted in 2007 and showed that phones can handle much more than email and phone calls. RIM faced numerous delays modernizing its operating system with the BlackBerry 10. During that time, it had to cut more than 5,000 jobs and saw shareholder wealth decline by more than $70 billion.

In the most recent quarter, RIM earned $98 million, or 19 cents a share, compared with a loss of $125 million, or 24 cents a share, a year earlier. After adjusting for restructuring and other one-time items, RIM earned 22 cents a share. Analysts surveyed by FactSet had been expecting a loss of 31 cents.

Revenue fell 36 percent to $2.7 billion, from $4.2 billion. Analysts had expected $2.82 billion.

RIM shipped 6 million BlackBerry devices, including 1 million on the new system. But RIM lost about 3 million subscribers to end the quarter with 76 million. It's the second consecutive quarterly decline for RIM, whose subscriber based peaked at 80 million last summer.

Bill Kreyer, a tech analyst for Edward Jones, called the decline "pretty alarming."

"This is going to take a couple of quarters to really see how they are doing," Kreyer said.

The company also announced that co-founder Mike Lazaridis will leave the company. He and Jim Balsillie had stepped down as co-CEOs in January 2012 after several quarters of disappointing results, but Lazaridis said he stayed on as vice chairman and a board director to help new CEO Thorsten Heins and his team with the launch of the BlackBerry 10. With that underway, Lazaridis plans to retire May 1. He said he has no plans to sell his 5.7 percent stake in the company.

In an interview with The Associated Press, Lazaridis said the board wanted both him and Jim to stay, but Lazaridis decided "it was the right time" to leave.

Heins, formerly RIM's chief operating officer, has spent the past year cutting costs and steering the company toward the launch of new BlackBerry 10 phones. Lazaridis said Heins has done an excellent job completing the BlackBerry 10 system and launching it around the world.

"The results speak for themselves," Lazaridis said.

Sterne Agee analyst Shaw Wu said RIM returned to profitability much sooner than expected. He said it was driven by higher gross margins, cost reductions and the sale of the new BlackBerry.

In a research note, Wu wrote that RIM "is here to stay with stabilization in its business and balance sheet" but said the key question remains whether the company can maintain momentum in an industry dominated by Apple and Google's Android software.

The Z10 has received favorable reviews since its release, but the launch in the critical U.S. market was delayed until late this month as wireless carriers completed their testing.

A version with a physical keyboard, called the Q10, won't be released in the U.S. for two or three more months. The delay in selling the Q10 complicates RIM's efforts to hang on to customers tempted by the iPhone and a range of devices running Android. Even as the BlackBerry has fallen behind rivals in recent years, many users have stayed loyal because they prefer a physical keyboard over the touch screen on the iPhone and most Android devices.

RIM, which is changing is formal name to BlackBerry, said it expects to break even in the current quarter despite increasing spending on marketing by 50 percent compared with the previous quarter.

"To say it was a very challenging environment to deliver improved financial results could well be the understatement of the year," Heins said during a conference call with analysts.

Heins said more than half of the people buying the touch-screen Z10 were switching from rival systems. The company didn't provide details or specify whether those other systems were all smartphones. He said the Q10 will sell well among the existing BlackBerry user base. It's expected in some markets in April, but not in the U.S. until May or June.

Diagnosed with cervical cancer and must have a hysterectomy


I have had 3 abnormal PAPS and 3 colposcopies due to HPV. The most recent colposcopy came back abnormal so my doctor scheduled a LEEP. My doctor was expecting to remove abnormal cells and let my immune system take over to kill the rest of the HPV that might still be there. Since I have a back condition and a back fusion, my doctor and I had discussed that it would be in the best interest of me and any possible child that I would not have children. So during the LEEP she decided to remove more cervical tissue than she normally would on a 35 year old that had never had children.

She found cancer. Stage 1a1 cervical cancer. I now have to have a hysterectomy, uterus and cervix. It will be vaginally and laproscopicly unless I have internal scar tissue from my back fusion. My GY oncologist is giving me the decision if I want to keep my ovaries or not. I also have to decide if the surgery will be done by my usual OB/GYN or the GY oncologist.

I?m worried about whether to have them take my ovaries or keep them.

I?m worried about which doctor to have do the surgery.

I?m worried about sex after the hysterectomy.

I?m worried about my bladder. They said I could have incontinence after surgery if my bladder is damaged during surgery.

I'm very sorry you're dealing with all of this. I had cervical adenocarcinoma IAI and was treated with a rad/hyst. Due to my previous cancer history (melanoma) we did go ahead and remove ovaries as well. Most of the time, ovaries are left with cervical cancer. It is a tough decision for sure. Do your research, get as many opinions as you need, then do what you feel is best for yourself long term.

As far as who to have perform the surgery, however, there's no question I'd go with the gyn/onc. They're the experts and would best be prepared in the (unlikely) event there are any surprises during the procedure. I'm sure if it were your gyn in this situation, she'd be having a gyn/onc do her surgery. It's no reflection on her, after all. It's about your receiving the best treatment possible.

After surgery, I was sent home with a catheter until my post-op appointment. I've had no bladder complications whatsoever. I have had constipation issues which I'd never had before, but I've been able to manage that. And, the experience with sexual function varies a bit with each sister. Some ladies say there is little change, while others do have more complicated issues. But, try not to worry so very much about those "down the road" issues right now. One step at a time!

Hi, I know just how scary it is to be given a cancer diagnosis. You are asking great questions and it is understandable the concerns that you have. I am not a healthcare professional so the answers I will give you are strictly my opinion. I do not have cervical cancer. I have Endometrial Stromal Sarcoma, Stage 2b. I had a LAH and BSO, lymph node bx and pelvic washing. Cancer was found after my hysterectomy in a fast growing fibroid. It was also found in my left ovary and tube. Different cancer from yours and my cancer grows with estrogen so the ovaries out was a MUST for me. Ask your doctors to explain the risk of recurrence if you leave your ovaries in. For cervical cancer there may not be a risk.

100% yes a GYN ONC should do the surgery OR assist the GYN.

If you are absolutely sure you do not want kids do the hysterectomy. There is another procedure though called radical trachelectomy that can save your uterus and you could stilll have children. As far as ovaries go you are young and if the GYN ONC says it is safe to keep the ovaries then keep them and avoid surgical menopause.

If you keep your ovaries you should not have problems with sex after you completely heal from a hysterectomy. I had a hysterectomy and ovaries removed. I also take oral treatment to prevent estrogen from producing from my adrenal glands (this is to help prevent a recurrence for my type of cancer) and I do have the vaginal dryness that comes from NO estrogen but I use special lubricants and we just take it slow and gentle. I have the same physical reactions as before surgery. You will too. The key is to relax. Tension and stress can increase pain in the pelvis.

Do not worry about your bladder. Do you have any problems now? I had minor issues before my surgey because my uterus was so enlarged. Now a year later all those issues have gone away. I do not even need a panty liner anymore. It can take a very long time for some woman to heal from surgery. I was so worried I would never feel good again. It just took me a lot longer than I expected. Try to remember that after your surgery that your body has been thru a trauma and needs time to adjust.

Everything is new and scary right now. Your fears are normal. Write down all your questions for the doctor and take someone with you on your visits so you can have an extra set of eyes and ears with you. Take notes too. Knowledge is power and the more you learn about your particular cancer and its treatment the easier it will become to adjust to the diagnosis. You will forever be a "survivor" now. You can do this you will find strength and courage you never thought you had.

I can cry reading this!!! I know what hell it is to go through this. I'm a little older - 49 - and already have 2 kids, but that in itself is hard. With 2 teenage daughters and facing whatever was going on "down there" and has a cancer spread has been hell. I started this almost 2 years ago with pap that came back abnormal glandular cells. I am HPV positive, but for YEARS had nothing at all abnormal, since college, so my regular doc had said I could go every 2 years for paps, so I waited and that next pap came back abnormal. I was terrified that I had made a mistake waiting the 2 years.

Next step were the biopsies and colposcopies. Twice over the following 6 months, but regular GYN didn't find anything. He said go back to year repeat paps since probably was false positive. I refused to wait so long and said repeat in 6 months, so he agreed. Again it was atypical glandular. I asked for a hysterectomy. He said no, we should try LEEP. I said NO... Hysterectomy. So he finally split the difference with me after we both gave valid points, and went with a cone biopsy. The look on his face when he came back with results 2 weeks later scared the **** out of me. He said "it's a good thing we went ahead and did that procedure". I was insistent on cold knife excision to get the clear pathology reports which I'm very thankful I did, because it came back adenocarcinoma in situ... but "to the margin". Off to the ONC GYN. He did a second cone biopsy to see if he could get a clean margin which would determine the type of surgery I could have. He didn't find any more lesions at all so scheduled me for a "simple" hysterectomy, laparoscopic robotic-assisted.

Next came the choice of ovaries. AT first I said rip them out I don't want to take any chances. But then I looked into it further. The ONC GYN pointed out that there were still some benefits of post menopausal ovaries, and studies that show that women who have ovaries out at a younger age can have a higher risk of cardiovascular disease later in life. I asked my regular GYN too, and both docs said they could support either decision and really didn't give any guidance. They both said that HPV and cervical cancer/precancer does not give you any higher risk of ovarian cancer. Since there is NO ovarian or breast cancer in my family, and there IS high blood pressure and heart disease, I decided to leave my ovaries.

I would 100% go with the ONCGYN for your surgery!!!! That's what they specialize in. As far as ovaries, look into the research, and look at your family history. The other thought I had about ovaries is that once they are gone, they are gone, but if you keep them, they still can come out later. If anything changes in family history, or if I have continued problems with cysts (have in the past), I can still get them out. It was a tough decision. I had my surgery 10 days ago on Mar 15th.

I don't have any bladder issues right now. I actually have no issues right now except with my back. I'm still healing from the LEEP so I can't have sex until my doctor clears me, which will hopefully be next week. Then my GY/onc will schedule the surgery for probably mid April

I haven't been given any other option but the hysterectomy. I can put it off and have a child right now, or I can have the hysterectomy now. We are in no position to have a child right now, so hysterectomy it is.

I was told I may have a catheter and I may not. It will all depend on the type of surgery they end up doing. With our house and pets and life I don't think I would be comfortable coming home with a catheter, but I guess I will cross that bridge if we come to it.

Thanks ladies. I think I wouldn't be as worried if the surgery was already scheduled and I was ready to roll, but I have no idea when it will be and I'm antsy. It is also frustrating because I can't post any questions in Pre-Op on the boards till I have a date set.

I did have clear margins with mine and I was very glad my OB/GYN decided to take more of the cervix than normal. All the cancer was found in that extra with clear margins. My cancer was 1mm. Awfully small and very glad they found it now. It could have gone years before it caused problems or was easily seen, because my doctor was telling me the new federal guidelines for PAPs had changed and I was going t be going to every 3 years. That could have been deadly for me.

I'm so sorry you are having to deal with this. I promise you will get thru this!

I'm 38 and just got the all clear yesterday. I no longer have endocervical adenocarcinoma 1A2 .. Since I have 2 children there was no doubt I was ready for the hysterectomy. As istillsmile pointed out there is the radical removal of just the cervix. Yes you can still bear children but at a much higher risk if you want that option. That's a personal issue. As far as ovaries my gyn/ onc said keep them if they look good. Your ovaries regulate many things. I kept mine. I would for sure use the gyn/ onc. Just safer that way. As far sex well can't say yet , I'm still healing. Bladder issues.... After having 2 8.5 lb babies vaginal I leaked only when my bladder was full and I did something like jump rope or jumped on a trampoline. Since my surgery I've had no problems. I only had robotic simple hyst with lymph node removal. I think the bladder issues can occur after a radical hyst but not all the time. I only had a cath for a few hours in the hospital after surgery. I have 7 dogs 5 inside so I know what u mean about coming home with a cath.

Also just wanted to add I'm in a clinical trial at md Anderson in Houston . It's a conservative surgery for early stage cervical cancer. That's why I only had the simple not the radical. If you had clear margins on the last cone sounds like you would qualify. Also they did give me the option to keep my uterus if I wanted more children.. You might want to check in on that too. This clinical trial is like what they did for breast cancer lumpectomies instead of radical mastectomies for early stage breast cancer. They hope this can become a standard of treatment for early stage cervical cancer.

It sounds like you and I had (having) a similar experience AustinRhea. I am still thanking God I had the strength and determination to push my ob/gyn for more extensive testing. I think he is too. As I said in previous post, he scared me - because he was clearly surprised, if not a little shaken, when he was telling me my results from the cone. I really like my regular gyn. Very sympathetic man, and we had a little conversation about intuition versus panic too. I told him I pushed because I just had a sense that something was very wrong.
I know what you mean about not having a date for surgery. when I first went to the ONC GYN I thought I was going to prepare for the hysterectomy. I didn't expect yet another cone and another waiting period for pathology results. It took 3 WEEKS (!) that last time since it was vacation week and the doctor was away. That was by far the WORST for me through all this. By the time I got a date and got to the actual hysterectomy, I'd had so my stress come and go, I was rather calm on surgery morning. The surgery turned out really easy for me. Wheeled into the O.R. at 8 a.m. and was on the road back home by 12:30 same day.
Funny thing... 2 weeks before surgery, I developed an abscessed tooth, and needed an emergency root canal. The pain was so bad while waiting for antibiotics to kick in, I actually ended up at the emergency room because even vicodin wouldn't touch the pain. Oxycodone also didn't do much. I joked about adding insult to injury to have to have a root canal and a hysterectomy in the same month, but at this point, it's clear that the root canal and tooth was by far worse than the hysterectomy!!!!

Synchrotron yields 'safer' vaccine

Producing vaccines against viral threats is a potentially hazardous business and that's why manufacturers have to operate strict controls to ensure that no pathogens escape.

British scientists have developed a new method to create an entirely synthetic vaccine which doesn't rely on using live infectious virus, meaning it is much safer.

What's more the prototype vaccine they have created, for the animal disease foot-and-mouth, has been engineered to make it more stable.

That means it can be kept out of the fridge for many hours before returning to the cold chain - overcoming one of the major hurdles in administering vaccines in the developing world.

The research, published in the journal PLOS pathogens, was a collaboration between scientists at Oxford and Reading Universities, the Pirbright Institute, and the UK's national synchrotron facility, the Diamond Light Source near Oxford.

Diamond is a particle accelerator which sends electrons round a giant magnetic ring at near light speeds.

The electrons emit energy in the form of intense X-rays which are channelled along "beamlines" - into laboratories where they are used to analyse structures in extraordinary detail.

Infectious

Synchrotrons have been used before to analyse viruses at the atomic level, but the technology has advanced considerably to enable scientists to create a stable synthetic vaccine.

"What we have achieved here is close to the holy grail of foot-and-mouth vaccines.

Unlike traditional vaccines, there is no chance that the empty shell vaccine could revert to an infectious form," said Dave Stuart, Life Sciences Director at Diamond, and MRC Professor of Structural Biology at the University of Oxford.

"This work will have a broad and enduring impact on vaccine development, and the technology should be transferable to other viruses from the same family, such as poliovirus and hand-foot-and-mouth disease, a human virus which is currently endemic in South-East Asia."

These human disease threats, like foot-and-mouth, are all picornaviruses.

Viruses are inherently unstable and fragile, but picornaviruses can be studied using X-ray crystallography.

This enables the protein shell of the virus to be analysed at the atomic level - something a billion times smaller than a pinhead.

Pathogen

As with any vaccine, the aim is to prompt the immune system to recognise this outer shell and destroy the pathogen before it has time to lock onto cells and infect them with its genetic material.

In this research the scientists created a synthetic viral shell, but lacking its pathogenic RNA interior - the genetic material the virus uses to replicate itself.

Crucially they were able to reinforce the structure of the viral shell to make it stronger, to improve the stability of the vaccine.

Pre-clinical trials have shown it to be stable at temperatures up to 56C for at least two hours. Foot-and-mouth is endemic in central Africa, parts of the Middle East and Asia, so this would be a significant improvement over existing vaccines.

With current foot-and-mouth vaccines it is difficult to distinguish between immunised livestock and those which have been infected.

That proved to be a major hurdle in controlling the foot-and-mouth outbreak in the UK in 2001 because it would have prevented the export of livestock.

Polio

But the synthetic vaccine should allow scientists to show the absence of infection in vaccinated animals.

"The foot-and-mouth-disease virus epidemic in the UK in 2001 was disastrous and cost the economy billions of pounds in control measures and compensation," explained Dr Bryan Charleston, Head of Livestock Viral Diseases Programme at the Pirbright Institute.

"This important work has been a direct result of the additional funding that was provided as a result of the 2001 outbreak to research this highly contagious disease."

The potential hazards of working with viruses was underlined in 2007 when the Pirbright laboratory site was identified as the source of a leak which led to an outbreak of foot-and-mouth disease.

Polio, another picornavirus, which exclusively affects humans, has been eliminated from nearly every country in the world, although it stubbornly persists in Nigeria, Pakistan and Afghanistan.

The need for secure vaccine production will become even more vital should polio be wiped out.

"Current polio vaccines, which use live virus for their production, pose a potential threat to the long-term success of eradication if they were to re-establish themselves in the population.

"Non-infectious vaccines would clearly provide a safeguard against this risk", said Dr Andrew Macadam, a virologist specialising in polio at the National Institute for Biological Standards and Control in Hertfordshire.

"This technology has great potential in terms of cost and biosafety.

"Any design strategy that minimises the chances of accidental virus release would not only make the world a safer place but would lower the bio-containment barriers to production allowing vaccines to be made more cheaply all over the world."
